Or

Sample letter for requesting a vacation for health reasons

Sometimes due to health reasons, you may be forced to use your vacation time to recover.  Usually this would only occur if your place of employment has no formal policy for sick time.  Here’s a template you can use to request vacation time for your health.

Recently I have been quite ill with ____(only disclose the details of your illness if you feel comfortable enough to, either way you still may be asked for a doctors letter to confirm your illness).
